RESCHEDULE REQUEST FORM

Below is the form that will be required for all reschedule requests including weather related. Please fill out all information. The form should be emailed to the League Administrator at michele.letina@gmail.com. Reschedules will not be considered final until the new game information has been posted on this website.

Each team is allowed one reschedule that is non-weather related.  The fee for the regular reschedule is $25.00.

BOTH TEAMS MUST AGREE TO RESCHEDULE.

NEW DATE MUST BE CHOSEN FROM AVAILABLE DATES SHOWN ON FORM.

ALL FIELDS MUST BE CONFIRMED WITH THE CLUB FIELD COORDINATOR BEFORE SUBMITTING.

The deadline for non-weather related reschedules to be requested is June 1st at midnight.
