RESCHEDULE REQUEST FORM

Below is the form that will be required for all weather related reschedule requests. Please fill out all information. The form should be emailed to the League Administrator at michele.letina@gmail.com. Reschedules will not be considered final until the new game information has been posted on this website.

BOTH TEAMS MUST AGREE TO RESCHEDULE DATE. YOU WILL HAVE ONE WEEK FROM THE WEATHER INCIDENT TO SUBMIT YOUR NEW DATE. AFTER THAT, A NEW DATE WILL BE ASSIGNED BY THE LEAGUE ADMINISTRATOR.

NEW DATE MUST BE CHOSEN FROM AVAILABLE DATES SHOWN ON FORM.

ALL FIELDS MUST BE CONFIRMED WITH THE CLUB FIELD COORDINATOR BEFORE SUBMITTING.
